2025 Student Influenza Immunization Clinic

The St. Croix County Public Health Department will be providing a free influenza
immunization clinic for school-aged children on Thursday, October 16th. If you would
like your child to receive the injectable influenza vaccine, a parent or legal guardian
must fill out a consent form, unless the student is 18 years of age. The parent does not
need to be present at the clinic if the child brings a signed consent form. Please return
the signed and dated consent form to school no later than Monday, October 13th.

The consent form and vaccine information sheet can be found below, on the school 
website, or at the school offices.

Schedule for Thursday, October 16th:
8:30 am Greenfield Elementary School
10:30 am Baldwin-Woodville High School
1:00 pm Viking Middle School

Immunization Requirements

Immunizations (also called vaccines) are one of the most important ways to protect children from diseases. Immunizations are especially important for school-aged children because children in school are regularly in close contact with others who may be able to spread these diseases. 

For this school year, all students will need the following immunizations OR an appropriate waiver.

Kindergarten-6th grade

  • 4 doses of polio

  • 4 doses of DTaP/DT/Td

  • 3 doses of hepatitis B

  • 2 doses of varicella (chickenpox) or history of disease documented by a qualified health care professional

  • 2 doses of MMR (measles, mumps, rubella)

What should I do?

Make sure your child is up to date on their vaccinations by checking your child’s immunization record. To get your child up to date on their vaccinations, make a vaccine appointment with their doctor or complete the Student Immunization Record form
